In order to help provide you with the very best service, please be sure to print or type all information legibly to ensure that your request is processed accurately and timely. Incomplete requisitions will often delay the processing of your request and the subsequent reporting of your results.
Customers will need to complete a separate Requisition Form for each patient and include it in the specimen kit provided. Before shipping, please verify that patient information and patient ID number on the form matches that on the specimen.
